Support Coordination
At InMotion Support Coordination we provide Support Coordination through the NDIS to assist people with the planning of their NDIS funding.
We will work with you and your network of support (partners, family, carers and so on) to understand your needs, provide a comprehensive plan and help you in your application for funding, assistive technology, home modifications and/or help you develop the skills you need to participate in LIFE!
Our experienced Support Coordinators will act on behalf of the participant to:
- Setting up and implementing your National Disability Insurance Scheme Plan
- Provide assistance with understanding your plan and the funding allocated
- Find new service provider options eg linking with respite services, occupational therapist, speech therapist, physio, assistive technology and home modifications
- Organise new services agreements for all supports
- Create service bookings with preferred providers
- Support with scheduled or unscheduled plan reviews
- Advocate on behalf of the participant
- Support Participants with achieving and implementing current NDIS goals
- Monitor quality of services
- Encourage participants choice and control over their current plan
- Provide assistance to access the community and mainstream supports
- Link with social networks
